How do I turn a care visit into a clear family handoff?

A useful care-visit handoff separates what someone reported, what was agreed and what still needs an answer. Name the next person and action only when those are actually known. An empty transport or contact field means the document lacks that detail; it does not establish that the person lacks support.

Still unknown: The next appointment date, who is available for transport and whether the contact sheet needs updating.

Source to check: The appointment letter kept in the parent's own folder. It is not attached to this general family note.

The handoff is complete as a communication artifact even though it contains unresolved questions. It tells the next reader what to do. Replacing “transport unknown” with “transport not arranged” would add a claim the author did not supply.

Assess the document's organization

Care-Plan Completeness is a guided self-assessment. You check the document, choose Recorded, Not recorded, Unknown or Not applicable for each criterion and enter its supporting wording. The tool counts your answers; it does not read or analyze a pasted or uploaded care document.
